Some Neurological Changes Need Medical Assessment First
With lower-back and leg symptoms, new bladder or bowel changes, numbness around the saddle/genital region, or significant/progressively worsening leg weakness can be associated with cauda equina syndrome and require urgent medical assessment.
With neck and arm symptoms, new problems with walking or balance, clumsy or weak hands, or broader/progressive neurological changes can indicate spinal cord involvement and require medical assessment.

FUNCTIONAL BELIEF SHIFT
The goal is not simply to make the travelling symptom disappear. It is to understand what is affecting function and progressively rebuild what you need to do.
Assess
Understand where you are now.
Plan
Identify current priorities.
Monitor
Observe response and progress.
Adapt
Modify as needs change.
Progress
Build towards meaningful function.
SCIATICA & RADIATING PAIN
- ✓When symptoms travel into an arm or leg, where you feel them is important—but it is only part of the clinical picture.
- ✓Core Functional Proposition
- ✓Rehabilitation needs to understand where symptoms travel, what influences them, whether strength or sensation has changed, and how everyday movement is being affected.
- ✓Core Belief Shift
- ✓Radiating pain is a symptom pattern—not a complete rehabilitation plan.
- ✓NICE describes radiculopathy as neck or low-back pain radiating into a limb that may be accompanied by tingling, altered reflexes or weakness. Sciatica specifically concerns symptoms affecting the lower limb; symptoms travelling from the neck into the arm represent a different cervical nerve-root pattern.
Sometimes Another Route May Be More Appropriate
Some Symptoms Need Prompt Medical Assessment
- Sudden severe pain with fever or feeling unwell
- Numbness in the groin or loss of bladder/bowel control
- Weakness that is rapidly worsening
- Pain after significant trauma or fall
